Anaphylactic shock

Heaviness degrees of the anaphylactic reaction/anaphylactic shock

4 heaviness degrees of an allergic/anaphylactic reaction are defined:

I. locally restricted skin reaction without a clinical relevance

II. General symptoms (dizziness, headache, anxiety, and others)+skin reactions (tumescence, pruritus and others)

III. additional: Blood pressure drop+heart lawn as well as gastrointestinal symptoms (nausea, abdomen spasms, vomiting and others), light respiratory troubles

IV. Additional: Bronchospasm (asthma attack) and shock, rare also Larynxedem, with a strong air need primarily when breathing in (typical murmur)

V. Breath and circulation interruption

Anaphylactic reaction

The sequence of following troubles which can increase and spread in a rapid velocity is typical of an anaphylactic reaction:
  • In the palms or under the soles of the foot it starts to prickle,
  • the face and followingly the complete body redden themselves, pruritus appears in consequence,
  • a heat feeling climbs in the complete body,
  • the pulse is faster
  • the blood pressure falls: Faint.
The anaphylactic shock represents the extreme variant of an allergic reaction. It is a form of the circulatory failure which is based on a surplus reaction of the body to an allergen on which the person became sensibilized, i.e. allergic. This shock shape most frequently appears after insect stings but also after pleasure of allergy triggering food or after medicine gift. Immune system is prerequisite for the anaphylactic reaction, earlier, of course the previous sensitization of the organism, the bodies always must already once with the substance in contact come have and the antibody must with and readiness education reacts have allergy. The frequency of the contacts with the allergenic substance, however, doesn't correlate with the heaviness of the reaction. By the blood pressure drop in the shock it blood vessels narrow made, which increases the heart rate it comes to the distribution of epinephrine and the blood flow the small so that ensure and of around, brain heart provides, i.e. the body nucleus is strengthened with blood now, a so-called "circulatory centralization" comes now to the temporary one. By the different mediators and the histamine fluid leaves, however, the blood vessels into the tissue, it comes to the body fluid deficit, the organ supply caves in, it comes to the heart circulation interruption. Anaphylactically shock represents an acutely life-threatening situation and must be immediately treated medically this one!

Therapy at the anaphylactic shock:
  1. !!!!!! Stopping a further allergen supply!!!!!!!!
  2. Epinephrine (SuprareninÒ): Immediately application as spray; Injection 1 mg of Suprarenin+ 9 ml physiological salt solution over 5 min. or dose resoled intratracheally
  3. Quick gift of volume in a sufficient quantity (normal: 2 l/30 min.)
  4. Cortison for the inflammation stop: Prednisolon 1.000 mg
  5. Antihistamines
    - H1-Blocker: z.B. Clemastin = TavegilÒ 4 mg
    - H2-Blocker: z.B. Cimetidin 400 mg
  6. At bronchospasm: Beta2-sympathomimetic as inhaling spray or theophylline 0.24, 0.48 g as a short infusion
  7. At continuing shock use of dopamine (Initial dose: 35µg / kg / Min. = 2,5 mg / 70 kg / Min.)
  8. At circulation interruption: Cardiopulmonary resuscitation
Persons at risk, such as wasp sting person suffering from an allergy, must permanently therefore lead a small emergency pharmacy with themselves at the wasp time. The so-called "Fastject-Injector" is included to inject itself epinephrine.
